As your preferred dental care provider, we make sure that at Centerville Family Dental, every visit to our office is memorable and stress-free. Please arrive at your appointment about 15 minutes early, and have your driver’s license and insurance card in hand. This will allow you to fill out your new patient forms easily – or, you can download them on our website and fill them out ahead of time, print them, and bring them with you.
For adults, we reserve time for a complete exam, which includes the following:
- A tour of our state-of-the-art office
- A review of your health history
- X-rays when necessary
- Imaging
- Hygiene evaluation
- A treatment and full cleaning
Our Centerville dentists will walk you through their findings and work closely with you to create a dental plan designed to meet your specific oral health goals.
For children, as long as the child is comfortable, the appointment includes time for the dentist and the child to get acquainted, complete an exam, and to clean your child’s teeth.
Please note: If you need to reschedule your appointment for any reason, we are happy to do so with 24 hours notice prior to your appointment.
